Аннотация
Введение. Используемые в настоящее время модели риска рака молочной железы (РМЖ) характеризуются невысокой прогностической ценностью.
Цель. Разработать многофакторные модели вероятности РМЖ, учитывающие как традиционные факторы его риска, так и количественный показатель маммографической плотности, и оценить их точность.
Материалы и методы. В исследование было включено 82 пациентки с гистологически верифицированным РМЖ и 269 женщин, не страдавших данной патологией. У всех пациенток выполнялась регистрация традиционных факторов риска РМЖ, а также оценивалась маммографическая плотность паренхимы и ее среднегодовое изменение. С целью классификации использовали бинарную логистическую регрессию, линейный дискриминантный анализ, иерархический кластерный анализ и анализ решающих деревьев.
Результаты. При корреляционном анализе были продемонстрированы слабые корреляции факторов риска РМЖ с фактом его наличия (r2 = 0,060-0,367). Единственным исключением явилась корреляция средней силы (r2 = 0,609) в анализе зависимости вероятности РМЖ от наличия в анамнезе биопсий молочной железы. В отсутствие динамики маммографической плотности наилучшие результаты продемонстрировала модель бинарной логистической регрессии (для всей выборки: чувствительность — 67,50 %, специфичность — 94,42 %; для пациенток с плотностью паренхимы ACR А-В — 75,86 % и 92,42 %, с плотностью паренхимы ACR С-D — 80,56 % и 89,26 % соответственно). При наличии динамики маммографической плотности чувствительность и специфичность модели бинарной логистической регрессии возрастали в полной выборке до 81,82 % и 97,62 % соответственно, для пациенток с ACR А-В и ACR С-D — до 100,00 % и 100,00 % соответственно.
Выводы. Комбинирование показателя маммографической плотности и его изменения с традиционными факторами риска РМЖ в модели бинарной логистической регрессии обеспечивает высокую точность прогнозирования (88,40-100,00 %) данного исхода.
