Аннотация

Введение. В данном систематическом обзоре проанализировано влияние биопсии сторожевого лимфатического узла (БСЛУ) на долгосрочные онкологические результаты у пациенток с клинически верифицированным поражением аксиллярных лимфатических узлов при раке молочной железы после неоадъювантной химиотерапии (НАПХТ) и достижение клинического ответа. Основная цель — оценить, обеспечивает ли БСЛУ при достижении клинического ответа в регионарных зонах сопоставимые показатели общей (ОВ) и безрецидивной выживаемости (БРВ) по сравнению со стандартной аксиллярной лимфодиссекцией (АЛД).

Материалы и методы. Поиск литературы проведен в марте 2025 г. в базах PUBMED и Google Scholar с использованием ключевых слов breast neoplasm, node-positive, neoadjuvant chemotherapy, long-term survival, а также ручным просмотром списков ссылок включенных статей. В обзор включались рандомизированные контролируемые исследования и ретроспективные когортные исследования, оценившие отдаленные показатели ОВ, БРВ и частоту аксиллярных рецидивов в группе пациентов с клинически верифицированным поражением аксиллярных лимфатических узлов и полным клиническим регрессом после НАПХТ в зависимости от выполненного хирургического вмешательства на регионарном лимфоколлекторе. Отбор литературы и извлечение данных выполнялись двумя независимыми рецензентами по рекомендациям PRISMA с оценкой риска смещений шкалой NewcastleOttawa.

Результаты. Из 341 найденного источника отобрано 20 исследований для систематического обзора. Пятилетняя ОВ в группах БСЛУ и АЛД составила 93,3 и 80,0 % соответственно, а БРВ — 84,7 и 73,2 % (p = 0,001). Частота аксиллярных рецидивов в обеих группах оставалась низкой и статистически не различалась (0,3–0,9 % для БСЛУ против 0,4–0,6 % для АЛД).

Выводы. Объем хирургического вмешательства в регионарных зонах не показал статистически значимого влияния на отдаленные показатели выживаемости пациенток при раке молочной железы с поражением лимфатических узлов и полной клинической регрессией после НАПХТ, однако требуются результаты крупных рандомизированных исследований высокого качества с отдельным анализом подгрупп.
