Abstract

Introduction. Pancreatic cancer is characterized by one of the lowest rates of early detection among gastrointestinal tract tumors. In the early stages, the disease proceeds without pronounced clinical symptoms, and at the initial visit, more than half of patients are diagnosed with a locally advanced or metastatic stage of the disease. The development and application of new approaches to detecting pancreatic cancer at early stages is of great clinical importance.

Aim. Development and application of a method for the early detection of pancreatic cancer based on an organizational experiment in St. Petersburg and the Leningrad Region.

Materials and Methods. The organizational experiment involved a two-stage system for identifying and further testing patients identified as at risk for pancreatic cancer. During the first stage, patients at high risk for pancreatic cancer were identified in primary care facilities. During the second stage, targeted testing was conducted at the Federal Center to confirm the diagnosis, followed by analysis of the results.

Results. During the organizational experiment between January 2019 and December 2023, 2.162 patients were examined at the first stage, 735 of them were allocated to the risk group, of which the diagnosis was confirmed in 182 (24.7 %) patients. Stage 0 of the disease was registered in 22.5 %, stage I — in 35.1 %, stage II — in 33.5 %, III — 6.5 %). In 2.4 % of patients, distant metastases were detected during examination notable that in these case size of the primary lesion in the pancreas did not exceed 2 cm.

Conclusion. The conducted organizational experiment demonstrated the effectiveness of the proposed method of early detection of pancreatic cancer both at the stage of primary health care and in a specialized center.
