Аннотация

В статье представлен мировой опыт циторедуктивных операций при раке яичников по литературным данным, а также оценен тринадцати летний опыт онкогинекологического отделения НМИЦ Н.Н. Петрова в лечении больных раком яичников (РЯ).

Материалы и методы: анализ результатов лечения за тринадцать лет предполагал сравнительную оценку результатов первичных и интервальных циторедуктивных операций у 213 больных РЯ. Для этого все пациенты, в зависимости от старта лечения, были разделены на две группы - группа больных распространенным РЯ, где лечение начиналось с неоадъювантной химиотерапии (ХТ) и группа больных распространенным РЯ, где на старте лечения выполнялась первичная циторедуктивная операция. В сформированных группах больных изучали отдаленные результаты лечения: бесплатиновый интервал (БПИ) и общую продолжительность жизни (ОПЖ).

Результаты: при изучении влияния количества курсов неоадъювантной ХТ на отдаленные результаты лечения установили, что наилучшие результаты по БПИ были получены в группе 3-х курсов неоадъювантной ХТ - БПИ составил 9,6 месяцев, в сравнении с 6,9 и 4,7 месяцах в группах 4-х и >5-ти курсов, хотя различия по группам не достигли статистически значимых различий (p=0,337). Увеличение количества курсов послеоперационной ХТ, в нашем исследовании, так же статистически недостоверно оказывало негативное влияние на выживаемость: больных из группы неоадъювантной ХТ с >3 курсами послеоперационной ХТ Бпи составил 9,6 месяцев, против 11,7 месяцев у больных с проведенными 3 курсами (р=0,787); ОПЖ составила 35,6 месяцев против 30,9 месяцев, соответственно (р=0,968). Та же тенденция прослеживалась и у больных из группы первичной циторедукции: в группе больных с >3 курсами послеоперационной ХТ БПИ составил 10,2 месяцев против 16,3 месяцев у больных с проведенными 3 курсами (р=0,312); ОПЖ составила 54,4 месяца против 48,7 месяцев, соответственно (р=0,435). Оценка выживаемости по методу Каплана-Мейера выявила статистически значимое улучшение показателей ОПЖ для больных из группы первичной циторедукции (медиана продолжительности жизни 53,7 месяцев; 95 % ДИ 41,9-73,6 месяцев) по сравнению с больными из группы неоадъювантной ХТ (медиана продолжительности жизни 33,0 месяцев; 95 % Ди 20,6-42,0 месяцев; p<0.000001).
