Аннотация
Успехи, достигнутые в терапии злокачественных опухолей у детей, привели к определенным проблемам у излеченных пациентов во взрослой жизни. Это связано с отдаленными последствиями противоопухолевого лечения. Особенно актуальным стал вопрос гонадотоксичности и снижения фертильности. На сегодня сохранение фертильности и детская онкология — это два «мира», которые постепенно сближаются. Варианты сохранения фертильности в перипубертатном и постпубертатном периоде хорошо известны и включают криоконсервацию яйцеклеток, эмбрионов или спермы. Методы сохранения фертильности в препубертатном периоде пока рассматриваются как экспериментальные. Многочисленные поиски активных исследований направлены на разработку технологий, которые позволят созревать незрелым яйцеклеткам и сперматозоидам и использовать их для рождения потомства в будущем. Кроме того, процедуры сохранения фертильности с медицинской точки зрения все-таки считаются необязательными, и родителям пациента нередко необходимо принять решение еще об одном, порой инвазивном, вмешательстве или преодолеть определенные этические проблемы.
