Аннотация

Введение. BRCA1-ассоциированный рак яичника (РЯ) характеризуется высокой чувствительностью к ДНК-повреждающим агентам, однако наиболее эффективная схема лекарственной терапии не определена.

Цель исследования. В рандомизированном исследовании сравнить клиническую эффективность комбинации «митомицин С и цисплатин» (МР) и «паклитаксел и карбоплатин» (TCbP) в неоадъювантной терапии распространенного BRCA-ассоциированного РЯ.

Материалы и методы. В исследование включены 98 больных распространенным РЯ, проходивших лечение в ФГБУ «НМИЦ онкологии им. Н.Н. Петрова» Минздрава России в период с 2019 по 2020 г. Были идентифицированы 16 (16%) носительниц патогенных вариантов BRCA1 и BRCA2. Больные были рандомизированы в соотношении 1:1 в экспериментальную группу для химиотерапии комбинацией митомицина С и цисплатина (МР) или группу контроля для химиотерапии по стандартной схеме комбинацией паклитаксела и карбоплатина (TCbP).

В группе МР все 8 больных распространенным BRCA-ассоциированным РЯ продемонстрировали объективный клинический ответ на лечение, тогда как в группе TCbP только 4 из 8 (50%) больных. Полный и умеренный патоморфологический ответ (СRS2 и CRS3) наблюдался у всех больных в группе МР и только у 5 из 8 (62,5%) в группе ТСbР.

Для оценки отдаленных результатов лечения включено 12 больных, проходивших лечение по схеме МР и 22 больные по схеме TCbP. Медиана безрецидивной выживаемости у больных распространенным BRCA-ассоциированным РЯ в группе MP была достоверно выше и составила 27,3 мес против 17,4 мес в группе TСbP (р=0,048).

Выводы. Комбинация митомицина С и цисплатина превосходит «золотой стандарт» неоадъювантной химиотерапии у больных BRCA-ассоциированным РЯ.
