Abstract

Introduction. Lipofilling is an effective method for correcting defects after breast cancer (BC) treatment. However, its widespread use is limited by variable fat resorption rates (30–70 %) and oncological safety issues related to the biological activity of adipose stem cells.

Aim. To evaluate the clinical efficacy and viability of "lipobolus"—a modified fat graft enriched with autologous platelet gel—for volume restoration and deformity correction in patients after radical BC treatment.

Materials and Methods. A literature review and a pilot case series (n=5) were conducted. Patients in remission (≥5 years) underwent implantation of a 50 mL lipobolus. The technique involved combining lipoaspirate with platelet-rich plasma (PRP) and calcium chloride to produce a dense, gel-like construct. Outcomes were assessed using ultrasound and clinical observation over a 3-month follow-up period.

Results. Graft viability was maintained in all cases, with satisfactory aesthetic outcomes achieved. One patient required a single seroma aspiration. No signs of fat resorption or specific complications (e.g., oil cysts, necrosis) were observed during the follow-up period.

Conclusion. The platelet gel-based lipobolus represents a promising technique for the single-stage restoration of significant soft tissue volumes with highly predictable results. A prospective study (MINIPIG) has been initiated to confirm long-term safety and efficacy.
