Abstract
Introduction. Soft tissue sarcomas (STS) are a rare and heterogeneous group of tumors. Blood vessel invasion has been described in 5% of cases, which was previously an indication for amputation. Thanks to the various methods of plastic and reconstructive surgery, indications for limb-salvage surgery are increasing.
Materials and Methods. 12 patients from June 1, 2020 until December 1, 2024 underwent surgical treatment at N.N. Petrov National Medical Research Center of Oncology: wide excision of a soft tissue sarcoma of the extremity with vascular resection and the reconstruction of the artery.
Results. Surgery was performed on the upper limb in 4 (33.3 %) cases, on the lower limb – in 8 cases. Artery resection was performed in 6 cases; in another 6 cases the artery and vein were resected, with no reconstruction of the venous vein. In 11 cases we grafted the autologous vein of the ipsilateral limb; in 1 case the saphenous vein from the contralateral limb was used. There were no cases of thrombosis of the artery neither in the early postoperative period nor during further observation. The average MSTSS was 80%, which is a good result.
Conclusion. Vascular invasion is not an absolute indication for amputation in patients with STS. However, appropriate planning and tactics under the conditions of the reference center are necessary to optimize treatment results.
