Аннотация

В обзорной статье рассматривается возможность использования хирургических радикальных и условно-радикальных методов лечения при злокачественных опухолевых поражениях плевры у больных раком легкого. Ни одно современное клиническое руководство не рекомендует их рутинное выполнение. Тем не менее, продолжаются попытки улучшить отдаленные результаты лечения больных этой категории с использованием полноценного хирургического этапа лечения. Результаты их неоднозначны, что связано с гетерогенностью описываемой группы и методологическими различиями. Большинство авторитетных экспертов сходятся в мнении о невозможности сделать обоснованные однозначные выводы о целесообразности, эффективности, безопасности и оптимальных объемах выполнения хирургических вмешательств у пациентов с опухолевыми поражениями плевры и отмечают необходимость проведения рандомизированных контролируемых исследований с учётом предполагаемого высокого потенциала использования у них современных комбинированных и комплексных методов лечения. При определении тактики лечения больных с опухолевым поражением плевры необходим индивидуальный подход с учетом наиболее значимых прогностических и предиктивных факторов, таких как: односторонний характер поражения плевры; локальное распространение (отсутствие признаков гематогенного и лимфогенного метастазирования, N-статус); пол, возраст, достаточные функциональные резервы; морфологическая структура, наличие драйверных мутаций; риск возникновения послеоперационных осложнений. Оптимизация использования арсенала существующих и перспективных методов лечения пациентов с немелкоклеточным раком легкого, имеющих опухолевые поражения плевры является одним из важнейших направлений современной онкологии, а многие аспекты рационального подхода к решению этой актуальной проблемы остаются нерешёнными и требуют дальнейшего изучения.
