摘要
Известно, что из всего спектра активирующих мутаций в онкогенах RAS/BRAF рак толстой кишки (РТК), возникающий у больных MUTYH-ассоциированным полипозом толстой кишки (МАП), демонстрирует наличие исключительно замены c.34G>T (p.G12С) в гене KRAS, относительно редкой в спорадических случаях РТК. Данное обстоятельство часто используется для выявления случаев МАП: детекцию KRAS G12C целесообразно использовать в качестве одного из критериев отбора больных колоректальным раком на диагностику этого рецессивного синдрома, как правило сопряженного с олигополипозом. В данном сообщении описана нетипичная находка: у больного с первично-множественным РТК и олигополипозом выявлена «двойная» мутация p.G12C (1 %) и p.G12D (12 %) в гене KRAS в опухолевой ткани. Последующий анализ MUTYH выявил компаунд-гетерозиготное носительство двух патогенных мутаций: c.933+3A>C и c.1187G>A (p.Gly396Asp). Оказалось, что в участке гистологического материала, подвергшегося микродиссекции, присутствуют одновременно тубуло-ворсинчатая аденома и рак: мутация p.G12C происходит из карциномы, а p.G12D — из аденомы. Нами также был выявлен случай BRAF V600E-позитивной аденомы у другого больного МАП. Таким образом, в то время как для MUTYH-ассоциированного рака характерна замена G12C KRAS, аденомы при МАП могут демонстрировать и наличие иных мутаций в генах каскада RAS-MAPK. Помимо непосредственного практического значения, наблюдение представляет фундаментальный интерес: по-видимому, аденомы с иными мутациями (не KRAS G12C) не подвержены или менее подвержены малигнизации при МАП.
