Аннотация

В настоящее время рак прямой кишки является одной из ведущих патологий в структуре онкологической заболеваемости и смертности, как в России, так и во всем мире.    

                                                                                                                             При этом, несмотря на совершенствование существующих и разработку новых  методов хирургического лечения и анестезиологического пособия, а также активным внедрением использования радио-и химиотерапии в качестве дополнения к хирургическому этапу специального лечения, рак прямой кишки, по-прежнему, характеризуется  весьма высокой смертностью среди данного контингента онкологических больных, и, в первую очередь, из-за того, что, примерно, в 30% случаев он диагностируются уже на весьма  поздних стадиях заболевания. И, при этом, комбинированная и комплексная  терапия до настоящего времени не способствует  улучшению отдаленных результатов специального лечения, которые, в конечном итоге, варьируются от полного ответа, в частности, при проведении радиотерапевтического лечения, вплоть до  абсолютной резистентности к его реализации.                                                                                                                                                            Вместе с тем, на сегодняшний день уже существуют исследования, подтверждающие связь между радиорезистентностью и экспрессией генов, которые индуцируют ответ на контрольную точку повреждения ДНК и увеличивают способность к репарации ДНК. При этом, молекулярные биомаркеры обладают вполне определенным потенциалом прогнозировать ответ на проведение, в частности, неоадъювантного химиолучевого лечения при раке прямой кишки.                                                                                                                        В данном, достаточно  кратком обзоре, рассматриваются вопросы эволюции и современного состояния неоадьювантной (предоперационной) химиолучевой терапии местнораспространненого рака прямой кишки (МРРПК). При этом, следует подчеркнуть, что рандомизированные исследования и метаанализы публикаций последних лет обосновывают целесообразность именно ее   применения у данной категории онкологических больных.                                                                                                          Также кратко рассмотрены современные стратегии лечения пациентов с местнорапространенным  раком прямой кишки и режимы радиотерапевтического лечения, применяемые в данной клинической ситуации, а также отражены перспективы использования молекулярно-генетических маркеров радиочувствительности и их возможное влияние на прогноз данного онкологического заболевания.
