Аннотация

В статье представлен анализ динамики заболеваемости злокачественными новообразованиями (ЗНО) и смертности от них. Предпринята попытка проанализировать причины роста или снижения заболеваемости и смертности от отдельных форм ЗНО. На основании анализа отечественного и международного опыта, высказаны предложения по планированию мер для дальнейшего снижения смертности и заболеваемости ЗНО в России. Смертность от ЗНО в России снижается в основном за счет снижения смертности от рака легкого и других форм рака, причиной которых является курение, а также рака желудка. Доминирующую роль в этом снижении играет первичная профилактика. Снижение смертности только от ЗНО сберегло жизнь, как минимум, 300 тысячам россиян, а также многим десяткам тысяч граждан других стран бывшего ссср, в которых также снизилась смертность от этого заболевания. Наблюдаемый рост заболеваемости ЗНО является результатом роста распространенности в популяции факторов риска ряда ЗНО. Однако немаловажную роль в этом росте играет применение для ранней диагностики современных высокочувствительных технологий, которые «выявляют» клинически незначимые образования, раннее не встречавшиеся в клинической практике. Для дальнейшего снижения смертности от ЗНО необходимо расширить спектр профилактических мер. Нужна программа вакцинации подростков против вирусов папилломы человека (ВПЧ). Вакцинированное поколение будет избавлено от рака шейки матки, а это одна из наиболее частых форм рака среди женщин. В ближайшие же 2-3 десятилетия необходимо проводить регулярный организованный скрининг для выявления инфицированности ВПЧ. Серьезный пересмотр нашей позиции по проведению скрининга позволит снизить смертность от рака шейки матки, ободочной и прямой кишки и, возможно молочной железы. Нужно внедрять методы, так называемой медикаментозной профилактики. Получены убедительные доказательства эффективности приема аспирина для профилактики рака толстой кишки и желудка, тамоксифена - для профилактики рака молочной железы. Будущее ранней диагностики за опухолевыми маркерами. необходимо апробировать в популяции эффективность определения опухолевых маркеров, например, циркулирующих свободных опухолевых ДНК и мРНК, для ранней диагностики рака. Это, конечно же, не значит, что мы должны отказаться от мер, направленных на ограничение курения и пропаганду здорового образа жизни. Снижение потребления алкоголя, изменение типа питания в сторону снижения потребления обработанных мясных продуктов и мяса и повышение потребления рыбы, овощей и фруктов, а также повышение физической активности, несомненно, приведет к снижению заболеваемости и смертности от многих основных форм ЗНО.
