Аннотация

В обзорной статье проведен анализ данных литературы и изложены современные представления о возможности, безопасности и эффективности проведения стереотаксической лучевой терапии больным с первичными (4-6%) и метастатическими (94-96%) опухолевыми поражениями позвоночника с учётом характера и степени распространения процесса, общесоматического и функционального статуса. Локальный контроль в течении 1 года при первичном (de novo) лечении пациентов составляет более 80%, при послеоперационном стереотаксическом облучении – 85-89%, при salvage-терапии - 66–92%, в качестве альтернативы хирургической декомпрессии 80-84%, при радиорезистентных метастазах 83-85%. Противоболевой эффект достигает 65–79% при salvage-терапии и 92-100% в остальных случаях. Может использоваться как облучение 16–24 Гр за 1 фракцию, так и фракционированные режимы 20–25-30 Гр за 2–3-5 фракций. Среди наиболее значимых осложнений метода - компрессионные переломы тел позвонков (11-39%) и миелопатия (1-5%). Показано, что решающую роль для успешного лечения играет тщательный отбор пациентов и использование современных, высокотехнологичных методик планирования. Особое значение имеет возможность проведения стереотаксической лучевой терапии у больных, которым ранее уже проводилось облучение. Метод значительно расширяет диапазон оказания специализированной помощи онкологическим больным, позволяет увеличить продолжительность жизни пациентов и улучшить её качество. Продолжается изучение преимуществ различных режимов фракционирования, значениях доз за фракцию. Методика требует высокой квалификации и достаточного опыта от медицинского персонала, а также полноценной оснащённости клиники диагностической и радиотерапевтической аппаратурой. Оптимизация методов лечения опухолевых поражений позвоночника является одним из приоритетных направлений современной онкологии, а многие аспекты рационального подхода к разрешению этой актуальной проблемы остаются нерешёнными и требуют дальнейшего изучения.
