Аннотация

Понимание молекулярно-генетического патогенеза наследственного ракового синдрома чрезвычайно важно для разработки персональных терапевтических подходов и для увеличения эффективности превентивных мер. На данный момент оптимальным решением для диагностики каузативных мутаций наследственного рака молочной железы (РМЖ) является тестирование таргетных мультигенных панелей с помощью высокопроизводительного секвенирования (NGS). Авторы сформировали NGS-панель из 31 гена, основываясь на их потенциальной причастности к формированию онкологической предрасположенности и встречаемости патогенных аллелей в российской популяции. В эту группу вошли «канонические» гены наследственного РМЖ (BRCA1, BRCA2, BRIP1, PALB2, TP53, ATM, NBN), «новые» гены, в которых относительно недавно были идентифицированы каузативные мутации (BLM, fAnCD2, POLE, FANCM, RAD51C, MRE11A, RECQL), а также некоторые другие гены, вовлеченные в репарацию ДНК, апоптоз и поддержание стабильности генома. Был выполнен скрининг мутаций у 94 пациенток с предположительно наследственным РМЖ неизвестной генетической этиологии, отбор пациенток был произведен согласно критериям соответствия наследственным опухолевым синдромам. Применение таргетной панели позволило с высокой долей уверенности назвать генетическую детерминанту заболевания у 21/94 (22,3%) больных, причем у 19 пациенток причиной заболевания были редкие мутации BRCA1 и BRCA2, не относящиеся к российским «фаундер» мутациям, а в оставшихся двух случаях были нарушены функции генов ATM (p.Glu73fs) и POLE (p.Leu1171fs). Данное наблюдение имеет большую клиническую значимость, поскольку является основанием для расширения диагностической панели, нацеленной на мониторинг лиц с повышенным онкологическим риском, и внедрения в клиническую диагностику мультигенного таргетного секвенирования.
