Аннотация

Попытки выявить в крови и иных биологических жидкостях изменения, сопутствующие ходу опухолевого процесса, имеют очень давнюю историю. Однако, реальная возможность мониторировать поведение опухоли в режиме «реального времени» появилась лишь с возникновением высокоточных методов ДНК-анализа. Следует разделить два направления в рамках этого раздела современной онкологии: 1) попытки раннего выявления опухолей среди условно здоровых лиц, а также 2) применение «жидкостной биопсии» у пациентов с уже выявленным онкологическим заболеванием для широкого круга задач: диагностики рецидивов, контроля эффективности лечения и т.д.

Можно констатировать, что первое направление пока не дало результатов, пригодных для массового внедрения в практику. Тем не менее, ошеломляют технологические достижения в совершенствовании аналитических показателей применяемых модификаций высокопроизводительного секвенирования. Вызывают интерес нестандартные подходы, основанные на использовании недавно открытых особенностей фрагментации циркулирующей опухолевой ДНК плазмы, комбинирование различных методов. Второе, менее амбициозное направление «жидкостной биопсии», оказывается значительно ближе к использованию в клинике. Этот подход уже широко применяется для такой актуальной задачи, как неинвазивное выявление мутаций T790M в гене EGFR у больных метастатическим раком легкого, получающих ингибиторы EGFR. Помимо практических достижений, в рамках изучения различных аспектов «жидкостной биопсии» удается получить ценные фундаментальные сведения, например, касательно паттернов (моделей или путей) возникновения резистентности к противоопухолевому лечению и динамики ее обратного развития после смены линии терапии.
